Employee's finger amputated in fall from elevated pallet

At approximately 4:00 p.m. on January 3, 1991, Employee #1, of Fedco, Inc., was on a pallet of boxes, elevated about 11 ft by a Yale industrial lift truck. He was storing the boxes atop a metal rack in the furniture warehouse when he lost his balance and reached for the yellow strap around one of the boxes. The strap broke and he started to fall, grabbing the metal support of the storage rack to break his fall. His right ring finger became caught in the metal rack's top slot support, and was amputated when he fell to the floor. The main causes of the accident were that the pallet platform lacked guardrails, and that neither a safety belt nor harness were provided.
