Investigation Summary
At approximately 11:50 a.m. on August 22, 1990, Employee #1, age 19, was using a 60-ton Diamond punch press to punch round pieces from a metal sheet strip coil. The pedal-activated punch press was set on single stroke manual operation. Employee #1 was reaching into the point of operation to remove some scrap metal when the ram came down and amputated the four fingers on his left hand, leaving only his thumb. He was taken to the UC-Irvine Medical Center, where he was hospitalized. The point of operation was not guarded. The punch presses at this site were owned by Cal Circle Company and operated by the employees of Alliance Metals, Inc. Employee #1, who was working alone at the time of the accident, was the son of the owner of the Cal Circle Company.
