Investigation Summary
At approximately 11:41 a.m. on September 25, 1991, Employee #1 was starting up a ladder of a scaffold work platform with a fire hose draped over his shoulder, which he was going to use to wash sawdust off the roof of a building. When he was on about the third rung from the top of the ladder, the fire hose started to slip. When he reached for it, his other hand slipped off the rung, causing him to fall. The scaffold work platform was 13 1/2 ft above the ground. Employee #1 fell approximately 8 to 9 ft and landed on his right heel, fracturing it. The employer was cited for two general violations not relating to the accident and one serious violation of T8CCR 3276(a)(1).
