Employee's finger smashed between boards

At approximately 11:10 p.m. on September 23, 1991, Employee #1 was feeding the finger-joint machine. As his right hand was placing a short piece of lumber onto the lug chain, another piece of lumber somehow got crossways on the chain and his right long finger was smashed between the two boards. Employee #1 instinctively jerked his hand loose, which caused the degloving of his finger. He was taken to the doctor's office, where the finger was reattached. Three weeks later the finger had not healed properly and had to be amputated to the first joint. Employee #1 was experienced and had been properly trained, including job safety analysis, safety check lists, and one-on-one training. Investigations revealed no violations of the safety and health regulations.
