Employee's fingertip amputated by filling machine

At approximately 6:30 a.m. on August 17, 1991, Employee #1, a mechanic for Pacific Coast Producers, trained in lockout procedures, decided to lubricate the valve shaft on a Scholle tomato paste filling machine with his finger instead of an extension tool while the machine was running. The machine had developed a steam leak and Employee #1 thought he could correct the leak by lubricating the valve shaft. He thought he could get it done before the machine recycled and the valve closed; however, the machine cycled, and the valve closed and pinched off his fingertip. The filling machine had been in operation for approximately 7 years. It normally runs continuously except on Sundays when it is shut down, taken apart, cleaned, and lubed.
