Investigation Summary

Investigation Nr: 170201131
Event: 02/04/1993
Employee injured when struck by backing forklift

At approximately 11:40 a.m. on February 4, 1993, Employee #1, a supervisor, was walking from the glue machine area out into the yard. A forklift was also going out in the same direction with a unit of lumber. After the forklift operator set the load down outside, he backed up in a circular arc to return and accidentally backed over Employee #1. The forklift wheel (pneumatic) backed up onto both legs and then had to be eased forward off them. Employee #1 was taken by ambulance to Ride Out Hospital in Yuba City, CA, where he was treated for two fractures of the left fibula and three lateral breaks of the right foot along with contusions and abrasions. Employee #1 was hospitalized for five days. He underwent surgery and was scheduled for additional surgery for hematoma of the right leg.
