Employee's finger amputated in fan-belt pulley

At 9:55 a.m. on April 14, 1993, Employee #1 had his right middle fingertip amputated when it was caught in the nip point of a tractor's fan-belt pulley. Employee #1 had checked the oil in the tractor and the operator had fueled it and returned to the driver's seat. The operator asked Employee #1 if it was "OK," and Employee #1 responded with "OK," referring to the oil level. Employee #1 reached under the hood to check the fan belt tension. As he put his fingers on the fan belt, the operator started the engine, which pulled Employee #1's middle finger into the fan-belt pulley and alternator, amputating the finger. Employee #1 was driven by another employee to Ride Out Hospital for treatment. Finger reattachment was not possible. No citation was issued. Both employees have been well trained and are long-time employees. The accident was caused by miscommunication. The tractor is a very large, high profile John Deere model 4240. The employer has a very well-devised safety program.
