Choker setter's fingers crushed by winch hook and roller

At 4:15 p.m. on July 20, 1995, Employee #1, a choker setter for an employer who logs by contract on U.S. Forest Service land, was seriously injured when his fingers became caught between a winch hook and the vertical right roller of a D6C Caterpillar skidder. Chokers are 5/8 in. cable and the winch line is 7/8 in.; a J type hook is attached to the winch line. Employee #1's left index finger was amputated at the middle knuckle, and his middle and ring fingers were crushed. The middle finger was reattached. Lack of communication between the operator and Employee #1 caused the accident.
