Investigation Summary
Employee #1 was working alone, taking rice samples at the 96 ft level of a rice milling building. He decided to cross through equipment to get to the separator room to clean up a pile of spilled rice. He jumped on an incline screw conveyor (RC-1 Auger) at a point that was 30 inches above the floor. His left foot suddenly went through a opening in the top of the screw conveyor cover and became caught in the moving conveyor. The screw auger was 6 in. in diameter (small). In one or two minutes, he pulled his foot free. Employee #1 had two lacerations to his left foot, lacerations of the extensor tendons, and a fracture of the second metatarsal with profuse bleeding. The hole in the screw conveyor should have been covered after a spout had been removed. Only red duct tape had been put over the hole.
