Investigation Summary

Investigation Nr: 170360358
Event: 10/18/1994
Employee injured when knocked from rolling stair

At 11:54 a.m. on October 18, 1994, Employee #1, of the Diamond Walnut Growers' bulk processing plant, was on a rolling stair taking a sample from an incoming truckload of walnuts. He was 8 ft 8 in. above the ground on the top landing, with the stair positioned parallel to the truck. Another truck was going the wrong way toward the in-gate and squeezing between employees' cars parked against the processing plant and the rolling stair. The other truck's driver said something to Employee #1 and continued by, but a tail-gate dump pin extending past the edge of the truck caught the stair. The pin moved the rolling stair 90 degrees, throwing Employee #1 to the ground. She suffered a fractured pelvis (both sides), a fractured right wrist, lacerations to her face, and a cracked front tooth. The other truck driver was driving in the wrong direction and too close to the projecting pin.
