Investigation Summary

Investigation Nr: 170617187
Event: 03/31/1995
Employee's finger amputated in tow chain

Due to heavy rains in May, the ground around the Broadview Co-op Gin was under water. Employee #1 was assigned to help a tractor driver tow wooden module pallets (32 in. by 7 ft) to higher ground. The driver backed the John Deere #6400 tractor for Employee #1 to hook up pallets to the draw bar of the tractor with chains. Employee #1 gave the driver the thumbs up signal to move forward. As the tractor took up slack, Employee #1's left little finger became trapped and the chain amputated the first joint of his little finger. Pictures were taken immediately because a large number of pallets still had to be towed out of the water. The gin manager who took the pictures gave them to OSHA.
