Employee injured in fall with work platform

At 10:42 p.m. on June 26, 1991, Employee #1 was standing in a prune bin elevated approximately 14 ft by a forklift that was operated by his foreman. He was using a high-pressure water hose to clean the exposed 2 by 6 bracings between the studs of the wooden warehouse walls. The prune bin was not secured to the forklift mast. It came off, causing Employee #1 to fall to the concrete floor. He suffered fractures of both hips and his left ankle, and internal injuries. Employee #1 later stated that he had washed down the walls before, but always from the floor. The employer was issued a regular citation for violation of T8CCR 342 (a), and serious citations for violations of T8CCR 3657(a)(2), 3659(a), and 3657(a)(3).
