Investigation Summary

Investigation Nr: 170168157
Event: 04/01/1991
Employee's head injured in fall from platform

At 3:12 p.m. on April 1, 1991, Employee #1 was installing wire mesh on the outside of a building in preparation for plastering. He was working from a 4 1/2 ft high A-frame scaffold platform that consisted of two 10 ft long 2 by 12 planks. He apparently stepped onto the end of the building, which was not supported, and fell. Employee #1 struck the back of his head, just below his hard hat straps, on a concrete block that was part of the building foundation. He sustained serious head injuries and was taken to Enloe Hospital, where he underwent surgery. The employer held weekly safety meetings and had a code of safe work practices.
