Investigation Summary

Investigation Nr: 170195515
Event: 08/23/1990
Night watchman on scooter injured in collision with forklift

At about 7:30 p.m. on August 23, 1990, at a wood remanufacturing plant (molding), Employee #1, a night watchman on the swing shift, was making his rounds on a small two-wheel Yamaha "RASS" motor scooter. As he left the north end of the chip bins, he traveled out into the path of a forklift, which was traveling at a normal speed, heading north in a forward direction, and was not loaded. The sun was still shining; therefore, lighting was not a problem. Employee #1 was hit by the right front side of the forklift and knocked forward. The forklift driver helped him off his bike and then called 911. Employee #1 was taken to Rideout Emergency Hospital and was treated and hospitalized for a compound fracture of the lower left leg and a broken left ankle.
