Investigation Summary
Employee #1 was on a Globe Industries bulk feed trailer, breaking up clumps of feed with an 8 ft long pipe so that it would flow out of the tank and through the stinger. He fell 12 ft 8 in. from the catwalk on which he was standing, and was hospitalized with multiple fractures. The catwalk did not have a guardrail or any other fall protection system.
