Investigation Summary
Employee #1 and a coworker were cleaning the parking lot and outside area of a warehouse. After collecting the trash in a given area, they deposited it in a steel hopper mounted on a forklift, and then drove the forklift to a new area. Employee #1 began driving the forklift at the same time he was raising the hopper, causing the vehicle to tip over. He jumped off the forklift, but the ROPS landed on his arm, fracturing it.
