Investigation Summary
At approximately 4:30 a.m., at the beet storage area where raw sugar beets were stored until they were required in the processing plant, Employee #1 relieved the flumer for the last rest break of the shift. His job was to wash the raw sugar beets into the water transport flume. It appears that a sugar beet blockage must have occurred at the mouth of the flume. As was customary at this workplace, Employee #1 apparently jumped on top of the blockage to break it loose instead of using a pole to loosen the beets from the other side of the flume's wall. Employee #1 fell into the flume and could not resurface, apparently because there were guard rails to keep the sugar beets submerged as they floated to the plant. There were no witnesses to the accident because the coworker was on rest break.
