Employee's hand injured when caught in chain and sprocket

Employee #1 was using a hand-held pump spray unit to clean and oil the chain and sprocket drive of a box conveyor. He was cleaning and servicing the box sealer machine when his right hand, which held the spray unit, struck the tension take-up sprocket. The conveyor was in the run mode and Employee #1's right hand suffered severe trauma when it became caught in the chain and sprocket drive. He was not trained in lockout requirements and procedures, or in the use of extension tools if the machine was required to be running. Employee #1 had been trained that the conveyor should have been stopped and not in the run mode during cleaning operations.
