Investigation Summary
On June 8, 1992, Employee #1 and a coworker were carrying a roll of insulation from one side of a steel-paneled roof to the other side. To do this they had to step over a 15 in. wide opening at the ridge. Employee #1 was walking in front, and somehow stepped into the ridge opening. He fell through the insulation of the roof, falling a total of 34 ft, and landed on some steel girts, which were lying on the concrete floor. Employee #1 was taken by ambulance to Delano District Hospital for treatment of a broken leg and head injury. He remained in the hospital until June 18, 1992.
