Investigation Summary
At approximately 1:00 p.m. on April 3, 1997, Employees #1 and #2, of Utah Pacific Construction, were using shovels to manually spread rock gravel inside a trench when the excavator bucket slid off the quick coupler pin and pinned both of them against the trench shield. Employee #1 sustained a serious hip injury and was taken to Northridge Hospital, where he was admitted for treatment. Employee #2 sustained bruises and was taken to Lancaster Community Hospital, where he was treated and released.
