Employee's thumb amputated in chain and sprocket drive

At approximately 1:15 p.m. on July 14, 1997, Employee #1, of Taylor Packing, Inc., was using a stick to clear carrots when he slipped and his right thumb became caught in the chain and sprocket drive. A portion of his thumb was amputated and he was taken by private vehicle to the Industrial Medical Group for treatment. The chain and sprocket drive was not guarded.
