Employee fracures her skull in a fall from a ladder

At approximately 2:00 p.m. on January 28, 2000, Employee #1, a warehouse worker, sustained a fractured skull when she fell approximately 8 ft and struck her head on a concrete floor. This accident occurred at the M.P. Environmental Services warehouse located in Bakersfield, CA. Employee #1 was climbing a Keller extension ladder to gain access to an 8.25-ft-high storage area, when the ladder slid from under her causing her to fall along with it. She was hospitalized. The base of the ladder was not placed with a secure footing.
