Investigation Summary
AT APPROXIMATELY 4:30 PM ON AUGUST 8, 1990, EMPLOYEE #1 WAS PREPARING TO ELEVATE THE REAR AXLE OF A GREAT DANE VAN WITH A HYDRAULIC JACK. THE EMPLOYEE PLACED A 4-INCH THICK, 6-INCH WIDE, 18-INCH LONG PIECE OF WOOD ON THE GROUND, SO THAT HE COULD SET THE JACK ON TOP FOR BETTER SUPPORT. HE THEN PLACED THE JACK ON THE WOOD AND ELEVATED THE REAR AXLE APPROXIMATELY TWELVE INCHES. AS EMPLOYEE #1 STARTED TO COME OUT FROM UNDERNEATH THE VAN, THE GROUND CAVED IN SEVERAL INCHES, THE JACK FELL, AND HE WAS CRUSHED BY THE REAR AXLE. EMPLOYEE #1 WAS HOSPITALIZED FOR HIS PELVIC INJURIES AND OTHER LOWER BODY INJURIES.
