Employee's leg broken when pipe rack collapses

At approximately 4:00 p.m. on June 28, 1990, Employee #1 and two coworkers were working at the Shell South Belridge lease in McKittrick, CA. Employee #1 and one of the coworkers were using a hoist to position 40 ft long sections of steam pipe, each weighing about 400 lb, into a pipe straightener for processing. After the pipes were straightened, they were placed perpendicularly across two A-frame pipe racks. The pipe to be located farthest from the straightener had two 1 1/4 in. holes drilled in one end of it. As Employee #1 and his coworkers were moving this pipe, it broke at the site of one of the holes. The additional weight of the pipe was suddenly transferred to the rack, causing the welds on the rack to break. The rack collapse onto Employee #1's left leg, fracturing his tibia.
