Employee's foot injured by falling valve

At about 3:00 p.m. on July 16, 1990, Employee #1, a machinist helper of Gary Drilling Company, was attempting to put a plug on the bottom of a kelly cock valve. The 1/2 in. diameter manila rope, which was used to secure the valve approximately 3 ft above the floor level, broke and the valve fell onto the employee's right foot. The employee sustained a serious injury to his right foot and was transported by private vehicle to Memorial Hospital in Bakersfield, where he was admitted for treatment until his release on July 19, 1990.
