Employee's face lacerated when struck by torquing tool

Employee #1, of Chrome Plating, Inc., was using a nut tightening tool to torque down a nut on a wheel polishing machine. The machine was not shut off as he worked and he was struck in the face by the tool. The machine was shut off by coworkers, and Employee #1 was transported to the hospital for treatment of facial lacerations. He later stated that this was the accepted and normal procedure for tightening the end nut.
