Investigation Summary
Employee #1, of the Fresno (CA) Parks and Recreation Department, was installing an auger head on the backhoe arm of a tractor. He had to lower the arm to install the hydraulic lines, and was standing at the arm's pivot point. When he reached up to the controls to lower the arm, he moved the wrong lever and the arm swung around and pinned him between the auger head and the rear of the tractor. He suffered multiple injuries and fractures, including a crushed hip and pelvis
