Employee's leg injured when struck by falling metal frame

Employee #1 was using a 5-ton bridge crane to lower a metal frame to the floor. One side of the frame was touching the floor when the chain sling holding the other side slid loose. The frame fell approximately 2 ft and when Employee #1 instinctively tried to catch it, the frame struck his leg. Subsequent inspection revealed that the frame edge had a burr, unnoticed by Employee #1, that caused it to tip and slip out of the sling. Employee #1 had been doing this type of work for this company for 2 1/2 years. The company had safety meetings with the section foreman every 5 days.
