Employee injured in fall through roof opening

At about 8:00 a.m. on September 25, 1991, Employee #1, the foreman of a two-man crew, was poking ceiling hangers (wire) through the roll formed metal roof decking on a new high school under construction. He lifted up a 4 by 8 plywood panel and stepped through a mechanical hole, falling about 14 ft to the floor. The panel was not secured and marked as required. It was determined that the prime contractor had previously secured a panel to this opening and marked it, but an unidentified person removed that panel.
