Employee struck in thigh by shaft and coupling

Employee #1 was operating a water pumping rig to test a client's well to determine its yield and the size of the permanent pump to be installed. While he was operating the equipment, the drive shaft sheared off, causing the nonreverse coupling to fly off the end of the gear head drive, which drives the water pump. The gear head drive is located between the diesel engine used to power the pump and the water pump. The coupling struck Employee #1's side and upper thigh. Note: Normally, when a drive shaft fails, it is below the ground. According to Employee #1, this was first time since he has been operating this type of equipment that a safety failure occurred above ground.
