Employee injured when struck by metal from sheared shim

While Employee #1 was operating a track press used to place and/or remove pins from the tracks of farm and heavy equipment, one of the retainer rings (washer) was caught between the pusher and the tooling house. This caused a portion of the metal retainer to be sheared off and strike Employee #1 in the lower chest. He was hospitalized. Evidence suggests that he may have been using an oversized retainer; however, this is not conclusive because there is also evidence suggesting that the retainers were the correct size.
