Investigation Summary
A power line worker was dispatched to a utility pole to replace a blown fuse. He donned his hardhat and safety glasses. With the bottom of a telescoping hot stick resting on the ground, the employee began extending the tool in a nearly vertical position to insert the replacement fuse. As he extended the last (bottom) section, the fuse fell off the tool's end attachment. After falling about 9.1 meters, the fuse struck the employee on the face, causing deep lacerations. He was hospitalized for his injuries.
