Employee injured in fall after slipping on wet floor

At 3:50 p.m. on June 28, 1991, Employee #1, the shift supervisor for Rogers Foods, came in from the mill to look for some parts that were on storage racks between the #4 and #5 garlic dryers. He was walking close by the #4 garlic drier, where the floor was always very wet, when he heard someone shout. Thinking they wanted him, he turned as he was walking, lost his balance, slipped, and fell. He suffered four fractured ribs and a punctured lung. Employees are required to walk along a designated walkway, or some other safe available route. The employer had suitable procedures for dealing with a wet process floor, but was cited for T8CCR 3203, a generally ineffective safety program, because the supervisor failed to follow safety procedures.
