Employee's finger amputated by printing roller

At approximately 1:30 a.m. on October 25, 1991, Employee #1 was operating a Copper's EG-100 Flexo machine. He was completing a two color printing set-up and was experiencing trouble with the quality of the ink application on the cardboard box being run. He set the machine in the jog/idle position, climbed on top of the machine, and reached in between the impression roll and the printing cylinder to touch the ink on the die (polymer). His right hand was pulled by and into an in-running roll and cylinder. Employee #1 suffered a broken index and middle finger. His ring finger was amputated at the second knuckle. Employee #1 was hospitalized.
