Investigation Summary
Employee #1, of Hershey Chocolate USA, was instructing a coworker on cleaning the syrup machine. The machine was running because the cleaner has to be introduced in the same way as the syrup. While pointing to a moving part, Employee #1 turned her head and her hand and arm became caught in the machine. She sustained multiple bruises and contusions to her hands and was hospitalized. The machine has sliding Plexiglas guards that were slid back at the time of the accident.
