Investigation Summary

Investigation Nr: 958017
Event: 03/03/1992
Employee's finger crushed between two racks, later amputated

Employee #1, of Foster Farms, was positioning metal racks containing turkey products. Each rack is equipped with four wheels, two stationary and two swivel. A handle on the swivel end is used to maneuver the cart. The cart fully loaded, as it was the day of the accident, weighs 1,500 lb. Employee #1 was attempting to position the cart between two other carts. Rather than having all his fingers inside the handle, Employee #1 had two fingers of each hand outside the handle. As he positioned the cart, he smashed the little finger of his right hand between the cart that he was positioning and a stationary cart. His finger was so severely smashed that his doctor decided to amputate it.
