Employee's fingers amputated by wire rope and pulley

At approximately 7:00 a.m. on February 20, 1992, Employee #1, of Danish Creamery Assoc., was helping a mechanic fix the motor to a dryer. Power had been locked out for this task. When the power was restored, Employee #1 was observing the sweep line through an opening (the cover had been removed) to see if it was operating properly. Inside the opening are two wire cables and pulleys. Employee #1 put his hands through the opening and his right little and ring fingers were amputated to the knuckle. The right middle finger and the left hand were also injured. Employee #1 does not remember why he put his hand through the opening. The employer was issued a serious citation for the violation of T8CCR 4070(f).
