Investigation Summary

Investigation Nr: 170196042
Event: 04/08/1990
Employee's foot fractured by falling food cart

At 10:45 a.m. on April 8, 1990, Employee #1, an inmate at Mule Creek State Prison, was receiving two hot food carts from the back of a truck at the kitchen of the minimum security dormitory. As the correctional officer/driver attempted to simultaneously level and lower the tailgate, it tilted down at the rear edge. The two carts rolled off, despite a coworker's attempts to hold them on the truck. One cart struck Employee #1 on the head and then fell on his right foot, fracturing a bone. The accident was the result of improper procedures and lack of equipment.
