Investigation Summary
At 3:00 p.m. on August 21, 1990, Employee #1, of Tuff Boy Sales, was seating the beads of a 16 in. tire on a 16 1/2 in. light truck rim. One of the beads and the sidewall ruptured, violently propelling the wheel 50 to 70 ft into the air. The wheel struck Employee #1 on the way up, fracturing his nose, both hands, and his left forearm, and causing other injuries. The accident was due to the installation of a tire on a non-matching light truck rim. Employee #1 had not received the required training.
