Employee injured when struck by sheetmetal coil

On June 27, 1991, Employee #1 was using a hoist to transport a 48-in.-high, 5.394-in-diameter, 2,000 lb, 20-gauge sheet metal coil to a decoiler on a pipe-making machine. The coil had been stored horizontally on a pallet and Employee #1 had used a cable to elevate both the coil and pallet to a vertical position. They had taken the cable off in order to remove the pallet when the coil tipped over. Employee #1's legs were injured, and he was hospitalized. The employer required supports to be used.
