Employee's fingers severed between descender and steel cart

At approximately 1:30 p.m. on January 8, 1991, Employee #1 was upstairs in the freezer department, sending down carts of ice cream for restocking. He put a cart into the descender and set the switch on AUTO so the ram would push in the cart all the way. Then he put the switch on MANUAL and pushed the DOWN button, which lowered the descender. At this point the cart, which was on a pivot, started to tip back out of the descender. Employee #1 put his hand on top of the cart to push it back while continuing to press the DOWN button. The descender dropped and caught Employee #1's fingers against the steel framework of the cart. He sustained partial amputation of three fingers on his right hand.
