Investigation Summary
On August 20, 1990, Employee #1, a journeyman bottle maker, dropped a nut while repairing a shut-down section of bottle-making machine C-1. Each section was 18 in. to 24 in. wide, and the standard practice was to shut down only one section at a time to keep the bottling operation in production. Employee #1 unintentionally reached into the adjoining section that was moving to retrieve the nut and the tip of his right middle finger was amputated.
