Employee injured when struck by falling pipe rack

On October 16, 1990, Employee #1, a millwright, was working with a three-man crew in the combuster at a 25 megawatt biomass electric utility plant. They were repairing the support for a bank of four 3 in. diameter tubes that carried hot water from the combuster. The 20-ton hydraulic jack they were using to support the cantilevered pipe rack slipped while Employee #1 was under it, and the rack fell on top of him. He sustained crushing injuries to his chest that caused his sternum to separate and he fractured his jaw in two places. The jack was atop a plywood panel that was resting on a pile of sand. The jack gave way when the sand shifted.
