Employee fell from loader tire while washing window

Employee #1 is the operator of a 950 Cat front end loader. While waiting to have a tire changed, Employee #1 climbed onto the rear tire of the loader to clean the rear window. While turning to sit and slide off the rear tire he caught his foot in the tire tread and lost his balance. He fell 4 feet to the pavement. Employee #1 suffered a fractured pelvis and a bruised elbow. The company performed an accident investigation which resulted in a change in the operating procedure requiring the use of extension tools for this job and prohibiting getting on the machine.
