Employee crushed to death by Caterpillar

Employee #1 was killed when the Caterpillar D6D he was operating ran over him. Employee #1 got off the Caterpillar to set his own chokers. He was behind the Caterpillar when it rolled backward over him. There was no witness to this accident. It appears that the brakes were not set and the blade was in the raised position. It is not certain where the Caterpillar was sitting--on a slight grade, at the top of the grade, or near the bottom. It is speculated that Employee #1 got off the Caterpillar without setting the brakes and with the blade in the raised position and that he was behind the Caterpillar when it rolled backward.
