Employee's fingertip amputated by tomato paste filler device

At 6:05 a.m. on August 2, 1993, Employee #1 amputated the tip of the left middle finger when it was caught by a tomato paste filler device. Employee #1 attempted to attach a 300 gallon bag to the filler machine, but was not holding the bag properly. Employee #1 held his hand close to the bag cap and pushed the cycle button. Consequently, his finger caught in a pinch point. Employee #1, feeling that he was not properly trained, said that the accident happened because it was only the second time he had operated that machine (2nd time he had pushed the cycle button). Employee #1 was taken to Ride Out Emergency where he was treated for losing the first joint of his left middle finger.
