Investigation Summary
On September 12, 1992, Employee #1, a millwright for Sierra Pacific Industries, Anderson, CA, was working the graveyard shift, He was attempting to repair a broken transfer chain on the infeed log deck to the sawmill. Employee #1 had placed a ladder against a catwalk under the conveyor, climbed to the catwalk, and then proceeded to climb over the motor drive to reach the broken chain. He grabbed hold of a piece of angle iron bracing to maintain his balance, but the angle iron broke off and he fell 15 ft to the ground. Employee #1 fractured his left knee and compressed a disc in his back. No fall protection was used. The employer was cited under 3210(b); repeat general, for failing to provide and use fall protection.
