Investigation Summary
Employee #1. of Timber Products Company, was a charger operator in a veneer mill. A veneer mill makes sheets of wood for plywood out of logs by running the logs through a lathe. The lathe peels sheets of wood off in approximately 1/8 in. or 1/4 in. thicknesses. The charger operator loads logs onto the apparatus that holds the logs up to the lathe knife. A set of arms (kickers) that push the logs onto the charger broke down. Employee #1 and a coworker were repairing the kickers. When Employee #1 tripped an electric eye that activates the charger, he was caught in the buttocks and raised in the air, and pinched between the charger's lower and upper arms. He sustained multiple fractures and was hospitalized. Employee #1 and the coworker did not lock out the charger, kickers, or related equipment before making the repairs. Employee #1 did not have a padlock to lockout the equipment.
