Investigation Summary

Employee #1 was on his first day on the job, spraying brush on steep terrain. He was carrying a 45 lb backpack sprayer that contained a 3 percent solution of Round-Up. He had been on the job for 10 to 15 minutes when he told his employer that he wanted to quit. The employer took his backpack and told Employee #1 to climb the hill to the road. Employee #1 had difficulty getting to the road and had to be assisted by coworkers. He lost consciousness while resting at the roadway, and never recovered. Employee #1 died. A toxicology report had not been completed at the time this report was filed. Employee #1 had indicated to the company owner that he had performed this type of work before.
