Employee injured in fall from stepladder

Employee #1 was nailing a truss in place when he fell from the 3rd or 4th step of a 6 ft high stepladder. There were no witnesses to the accident, and he did not remember what happened. Employee #1 later stated that he remembered nailing on a truss, and the next memory was lying injured on the floor, being assisted by coworkers. The stepladder was in very good condition.
